Reporting Fraud, Waste, and Abuse
Bark is committed to preventing and addressing fraud, waste, and abuse (FWA).
Fraud means intentionally providing false or misleading information to receive money, services, or benefits improperly.
Waste means using healthcare or Medicaid resources unnecessarily or inefficiently, resulting in avoidable costs.
Abuse means practices that are improper or inconsistent with accepted standards and may result in unnecessary costs or payments.
If you suspect fraud, waste, or abuse involving Bark’s services or Medicaid-funded products, please report your concern to [Bark reporting contact/method].
You may also report suspected Medicaid fraud, waste, or abuse directly to your state Medicaid agency.

You do not have to report your concern to Bark before reporting it to an outside agency. Bark prohibits retaliation against anyone who raises a concern in good faith.
Complaints About Bark Products or Services
Bark is committed to providing safe, reliable, and high-quality products and services. We want to hear from you if you have a concern or are dissatisfied with a Bark product, service, or your experience with Bark.
How to Make a Complaint
You may make a complaint about any aspect of Bark’s products or services, including concerns about:
- the quality or reliability of a product or service;
- customer service or support;
- delays or problems receiving or using a Bark product or service;
- the way you were treated by a Bark employee or representative;
- accessibility or accommodations;
- privacy or confidentiality; or
- any other concern about your experience with Bark.
Complaints may be made by the person receiving Bark services or by a parent, guardian, caregiver, authorized representative, or other person acting on their behalf.

Please provide enough information for us to understand your concern and how we can contact you. You are not required to use any particular form or wording to make a complaint.
What Happens After You Make a Complaint
Bark will review your complaint and, when appropriate, contact you for additional information. We will work to address complaints promptly and fairly and will let you know the outcome or any next steps, as appropriate.
You will not be denied services, treated differently, or retaliated against because you made a complaint or raised a concern in good faith.
Assistance and Accessibility
If you need assistance making a complaint, or need an accommodation or communication assistance to do so, please let us know. Bark will provide reasonable assistance to help you communicate your concern.
Medicaid or Other Program Complaints
If you receive a Bark product or service through Medicaid, a state or local agency, health plan, waiver program, or other publicly funded program, you may also have the right to make a complaint or grievance directly with that program or agency. Making a complaint to Bark does not prevent you from using any complaint, grievance, appeal, or other rights available through the applicable program.
Notice of Nondiscrimination and Civil Rights
Bark Technologies complies with applicable federal and Wisconsin civil rights requirements. Bark does not exclude individuals from participation in, deny them the benefits of, or otherwise discriminate in its programs or services on the basis of race, color, national origin, sex, age, or disability.
This commitment applies to services and activities provided by Bark directly and, where applicable, through contractors or other entities that provide services on Bark’s behalf.
Accessibility and Communication Assistance
Bark is committed to providing individuals with disabilities meaningful and effective access to its programs and services.
Bark will provide appropriate auxiliary aids, services, or other reasonable assistance when necessary to ensure effective communication and access for individuals with disabilities, at no cost to the individual.

Bark provides appropriate language assistance to individuals with limited English proficiency when needed to access Bark’s programs and services. Language assistance is provided free of charge.

Filing a Civil Rights Complaint
If you believe Bark has failed to provide appropriate language or accessibility assistance or has discriminated against you on the basis of race, color, national origin, sex, age, or disability, you may submit a complaint to Bark using the contact information above.
Bark prohibits retaliation against anyone for raising a civil rights concern, filing a complaint, or participating in the investigation of a complaint.
